Transition Stabilization
Safety, confidence, and learning what's safe
The first 3 weeks are the most sensitive. Your Physiotherapist will:
- Help you understand your specific cardiac or medical restrictions
- Monitor your heart rate and symptoms during activity
- Teach you safe pacing strategies for daily tasks
- Start gentle walking and movement at appropriate levels
- Reassure you about what's normal vs concerning
This phase is about building trust in your body again while protecting your recovery.
Functional Reconditioning
Rebuilding strength and activity tolerance
As your body adapts, your Kinesiologist helps you:
- Progressively increase walking distance and duration
- Build strength for daily activities
- Improve endurance without overdoing it
- Return to household tasks and light community activities
- Continue learning to pace yourself
This phase is about steady, monitored progression.
Independence & Handoff
Confidence and transition
Your Rehab Assistant provides:
- Continued practice and reinforcement
- Confidence building for independent activity
- Monitoring for any late concerns
- Support transitioning to formal cardiac rehab or self-management
This phase ensures you're ready to continue safely on your own or in another program.
Who This Program Is For
This Program May Be Right For You If:
- You recently had a cardiac event (heart attack, heart failure, cardiac surgery, etc.)
- You were hospitalized for a serious medical condition and feel deconditioned
- You're medically stable with clearance to begin activity at home
- You're not yet ready for — or don't have access to — formal cardiac rehabilitation
- You feel fearful or uncertain about physical activity
- You want structured, expert-guided recovery at home
This Program May Not Be Suitable If:
- You have unstable cardiac or medical conditions
- Your physician has not cleared you for home-based activity
- You have severe cognitive impairment preventing safe participation
- Your condition is related to an ICBC motor vehicle claim (we have a separate program)

After the 12 Weeks
At the end of the program, your care team will discuss next steps with you. Typically:

You're ready for formal cardiac rehab
We'll help coordinate your transition to a cardiac rehabilitation program.
You'd benefit from continued support
We'll discuss options for ongoing monitoring or check-ins.
