    Why This Program Exists

The Challenge Many Face After Cardiac Events or Hospitalization

Whether you've had a heart attack, heart failure, cardiac surgery, or another serious medical event, the weeks at home are critical — and often the most difficult:

Severe fatigue that makes even basic activities exhausting

Fear of physical activity and worry about triggering another event

Uncertainty about what's safe and how much is too much

Difficulty pacing yourself between activity and rest

Long waits or difficulty accessing formal cardiac rehabilitation

Loss of confidence in your body and what it can do

    Our Solution

Post-Hospital & Complex Discharge Home Recovery brings structured, expert-guided rehabilitation directly to your home during the critical recovery period — so you can rebuild strength safely, regain confidence, and return to the activities that matter to you.

This isn't generic exercise therapy. It's a carefully designed, three-phase program that provides the right type of support at the right time as your body recovers and rebuilds capacity.

    Your Care Team

    How Your Recovery Progresses

Three carefully designed phases that evolve as you recover.

    Phase 1Weeks 0–3

    Transition Stabilization

Safety, confidence, and learning what's safe

The first 3 weeks are the most sensitive. Your Physiotherapist will:

    • Help you understand your specific cardiac or medical restrictions
    • Monitor your heart rate and symptoms during activity
    • Teach you safe pacing strategies for daily tasks
    • Start gentle walking and movement at appropriate levels
    • Reassure you about what's normal vs concerning

This phase is about building trust in your body again while protecting your recovery.

    Phase 2Weeks 3–9

    Functional Reconditioning

    Rebuilding strength and activity tolerance

As your body adapts, your Kinesiologist helps you:

    • Progressively increase walking distance and duration
    • Build strength for daily activities
    • Improve endurance without overdoing it
    • Return to household tasks and light community activities
    • Continue learning to pace yourself

This phase is about steady, monitored progression.

    Phase 3Weeks 9–12

    Independence & Handoff

    Confidence and transition

    Your Rehab Assistant provides:

    • Continued practice and reinforcement
    • Confidence building for independent activity
    • Monitoring for any late concerns
    • Support transitioning to formal cardiac rehab or self-management

This phase ensures you're ready to continue safely on your own or in another program.

    Safety & Communication

    What's Normal During Cardiac Recovery

    • Mild fatigue after activity that improves with rest
    • Gradual improvement in walking tolerance
    • Some good days and some harder days
    • Increased confidence over time

When We Stop & Get Help Immediately

Your care team will stop activity and contact your physician or 911 if you experience:

    • Chest pain, pressure, or discomfort
    • Severe shortness of breath or dizziness
    • Irregular heartbeat or palpitations
    • Any concerning cardiac symptoms

Your safety and medical stability are always the priority.

You've rebuilt confidence and capacity — you're safe to continue on your own.

    2.

You're ready for formal cardiac rehab

We'll help coordinate your transition to a cardiac rehabilitation program.

    3.

    You'd benefit from continued support

We'll discuss options for ongoing monitoring or check-ins.
