This is not open-ended therapy. It's a focused, time-limited care package designed to help you safely rebuild strength, mobility, and confidence during the highest-risk recovery period.

Who This Program Is For

How the Program Works
A structured 6-week timeline for your recovery.
Assessment & Planning
Your Physiotherapist comes to your home to:
- Assess your current mobility, strength, and safety
- Evaluate your home environment
- Understand your goals and concerns
- Create your personalized recovery plan
Regular Support Visits
Your Rehab Assistant visits regularly (typically 1-2 times per week) to:
- Practice safe walking and transfers
- Guide you through exercises and functional activities
- Monitor how you're responding
- Adjust activities as you progress
Visits are front-loaded — more frequent early on when you need the most support, then gradually reduced as your confidence improves.
Progress Review
Your Physiotherapist returns to:
- Reassess your progress
- Update your plan if needed
- Prepare you for next steps after the program
Safety & Communication
What's Normal During Recovery
- Mild fatigue after activity
- Temporary muscle soreness (resolves within 24-48 hours)
- Gradual improvement in what you can do
When We Pause & Reassess
Your care team will stop activity and contact the Physiotherapist if you experience:
- A fall or near-fall
- New dizziness, chest symptoms, or confusion
- Pain that gets worse or doesn't improve
Your safety is always the priority.
Documentation & Guidance You'll Receive
Throughout the program, you and your family will receive comprehensive documentation.
Physiotherapy Assessment Report — detailed findings and recommendations
Home Safety & Mobility Summary — observations about your home environment
Individualized Recovery Plan — your goals, exercises, and safety guidelines
Caregiver Guidance Sheet — how family can help, what to watch for
Mid-Program Progress Summary — how you're doing
Discharge Summary — final report and next-step recommendations
With your consent, these documents can be shared with your doctor, case manager, or family members.
After the 6 Weeks
At the end of the program, your care team will discuss next steps with you. Typically, one of three things happens:
You've recovered well and are independent
No further therapy needed — you're safe and confident at home.
You'd benefit from ongoing light support
Optional once-weekly Rehab Assistant visits can continue for routine, confidence, and maintenance.
You need different or ongoing care
We'll help you transition to another appropriate program or community service.
There is no obligation to continue beyond 6 weeks. The program is designed to be time-limited, with clear outcomes and honest recommendations.

Why a Package Price?
Instead of paying per visit, this program is priced as a complete care package so you know exactly what to expect — no surprise costs, no open-ended billing.
Optional: Weekly Maintenance Support
Available only after completing the 6-week HomeSafe Recovery Program.
What It Is
Once-weekly Rehab Assistant visits for clients who:
- Are mostly independent but benefit from ongoing supervision
- Want routine and accountability to maintain mobility
- Don't need regular Physiotherapy reassessment
What It Includes
- 1 in-home Rehab Assistant visit per week
- Supervised walking, balance, and functional practice
- Monitoring for any concerns or changes
- Communication with Physiotherapist if issues arise
Billed per visit or in short rolling packages. No long-term commitment required.
This option maintains light support without creating unnecessary dependency or cost.
